Traumatismes vertébro-médullaires balistiques et insécurité au Burkina Faso : cas des blessés pris en charge aux urgences du CHU Tengandogo.
Introduction: La situation sécuritaire à laquelle fait face le Burkina Faso depuis 2015, engendre des traumatismes divers pour lesquels le service de neurochirurgie est sollicité à bien divers aspects. L’objectif est d’analyser la prise en charge des patients victimes de traumatisme vertébro-médullaire dans notre service au regard de l’insécurité. Méthodologie: il s’agissait d’une étude rétrospective portant sur les patients reçus aux urgences neurochirurgicales du CHU-T suite à une attaque terroriste au cours du mois de Mai 2022 à juin 2025. Résultats: 12 personnes blessées ont été prises en charge, dont neuf (9) hommes, d'un âge moyen de 35,8 ans. Le délai moyen d'admission aux urgences neurochirurgicales était de 03 jours. Cinq blessés provenaient de la région Est. Sept patients présentaient des polytraumatismes. Le segment vertébral atteint était dominé par le segment dorsal 58,3 % des cas, suivi du segment lombaire (35 %). La moitié des patients étaient classés ASIA A. Tous ont bénéficié d'un débridement chirurgical de la plaie, avec ou sans extraction du projectile. Sept patients ont été stabilisés par ostéosynthèse. L'évolution a été marquée par une amélioration du score ASIA chez quatre patients et un décès. Conclusion : le délai de prise en charge des blessés est long et rend la notion d’urgence obsolète et aggrave le pronostic des patients. Il est donc essentiel de créer des centres de neurochirurgie spécialisés dans différentes régions du pays afin d’améliorer la chaîne d’évacuation des blessés.
